Tuition
The fee for this 16-day course is:
$3,250.00 CDN for NDTA Members
$3,540.00 CDN for non-NDTA Members
Course fees include reference materials, a lab manual and refreshments. Lunch is not included. All tuition fees must be paid in Canadian dollars. Outside of Canada, please obtain a bank draft in Canadian funds. All payments must be received within 2 weeks of registration and by August 30, 2019.

Payment Options
- Payment can be made by credit card (Visa and Master Card are accepted)
- Payment can also be made by cheque. Option to pay by cheque will only be available until August 23, 2019. Following that date, all payments must be made by credit card
- Cheque payments must be received by August 30, 2019
- Cheques should be made to the order of University Health Network and sent to Conference Services – UHN, 550 University Avenue, Room 5-121, Toronto, ON, M5G 2A2

Cancellation Policy
Cancellations must be received in writing and all refunds are subject to a $200.00 administrative fee. After August 30, 2019 and in the event that a replacement cannot be found, no refunds will be granted.
