Internet Explorer

  1. On the Tools menu, click Internet Options, then click the Security tab.
  2. Click the Internet Zone.
  3. If you do not have to customize your Internet security settings, click Default Level. Then do step 4.
  4. If you have to customize your Internet security settings, follow these steps:
    a. Click Custom Level.
    b. In the Security Settings - Internet Zone dialog box, click Enable for Active Scripting in the Scripting section.
  5. Click the Back button to return to the previous page, and then click the Refresh button to run scripts.


  1. In the Chrome address bar, type chrome://settings/content/javascript.
  2. Adjust the toggle so that javascript is allowed.
  3. Close the Settings tab and Refresh the page.


Application Requirements

  • Copy of Current License
  • Proof of Current Malpractice Insurance
  • Proof of NDTA membership if applicable, so you can qualify for the lower tuition rate

Application Status

Applicants will be contacted via e-mail with application status: accepted, declined or placed on waiting list. The e-mails will be sent out by September 10, 2020. Please reply to the email to confirm your participation. If you are not accepted, the paid registration fee will be fully reimbursed.

Registration Rates

The fee for this 16-day course is:
$3,250.00 CDN for NDTA Members
$3,550.00 CDN for non-NDTA Members

Course fees include reference materials, a lab manual and refreshments. Lunch is not included. All tuition fees must be paid in Canadian dollars. Outside of Canada, please obtain a bank draft in Canadian funds. All payments must be received within 2 weeks of registration and by September 10. 2020.

  • Payment can be made by credit card (Visa and Master Card are accepted)
  • Payment can also be made by cheque. Option to pay by cheque will only be available until September 10, 2019. Following that date, all payments must be made by credit card

Cancellation Policy

Cancellations must be received in writing and all refunds are subject to a $200.00 administrative fee. After September 10, 2020 and in the event that a replacement cannot be found, no refunds will be granted.
