D.E. Systems' myConferenceSuite Registration System is responsible for personal information under its control. Collection of personal information by myConferenceSuite will be limited to what is necessary for the purposes of registration for the event.

 

When we use trusted third parties to act on our behalf by performing such functions as processing credit card payments, contractual or other appropriate means are used to ensure compliance by such third parties with this Policy and all applicable privacy laws.

 

Personal information will not be used or disclosed for purposes other than those for which it was collected, to process the registration data to a given event.

 

We will keep your personal information as accurate, complete and up-to-date as necessary for the purposes for which it is to be used.

 

The Policy is subject to PIPEDA and/or any other applicable privacy laws and myConferenceSuite reserves the right to change it at any time.

Collection and Use of Personal Information

CHF Canada uses the information collected on this form to provide you with information and services related to the:

• CHF Canada Member Education Forum,
• CHF Canada Co-op Managers Conference, and
• CHF Canada Annual Meeting and associated social and award events.

being held during the period of June 10-13, 2021, inclusively. We also use the information in preparing statistical reports about attendance at these events, for internal use.

By proceeding with this registration, you confirm:

• that, if you are registering for someone other than yourself, you have obtained the permission of the person(s) you are registering to use their email addresses for registration of one or more of the above-noted events
• that CHF Canada can use this email address to provide information and services related to one or more of the above-noted events with this registrant directly
• that you understand CHF Canada will maintain physical, electronic and procedural safeguards in compliance with applicable law and any CHF Canada policies to guard personal information from unauthorized access or alteration, and
• that you have reviewed this information with the registrant prior to obtaining their consent.

Registrants will also be advised directly when they login to their events that their email address will be kept separately from any other data for up to five years after the above-noted events, for use in contacting them with respect to their participation in CHF Canada-sponsored surveys and consultations, and to provide them with information about other services that may be of interest to them. This information will be stored on a secure server with restricted permissions and used only for the purposes described here. If requested in writing after June 15, 2021, this email information can be destroyed sooner.

All other personal information collected in this registration process for these described events will be stored securely with restricted access until such time as it can be destroyed, which will be by no later than November 30, 2021.
