Introduction to Myofascial Pain Syndrome and Trigger Points

Associate Professor and Clinician Scientist, Department of Medicine, University of Toronto; Affiliate Scientist, Pain Research Institute, Toronto Rehabilitation Institute

This session will provide an overview of the magnitude of the problem of chronic myofascial pain syndrome, explain the pathology of muscles and fascia, teach the diagnostic criteria for myofascial pain syndrome, and show a novel approach for myofascial pain disorder diagnosis which includes imaging, biomarkers and clinical examination.

Participants will be able to:
1. Recognize the prevalence and complexity of myofascial pain syndrome
2. Identify gaps in diagnosis and treatment
3. Identify the need for objective markers that spans disciplines and the role of ultrasound as a potential marker
4. Summarize the latest research

Assessment, Diagnosis and Treatment of Myofascial Pain Syndrome

This session will provide an overview of the most common muscle groups affected by myofascial pain syndrome (neck and lower back), will explain practical aspects of taking history and physical examination of a patient with chronic myofascial pain.  It will also summarize evidence-based treatments for myofascial pain syndrome.

Participants will be able to:
1. Recognize the muscle systems and their dysfunctions and the presence of trigger points in specific body parts such as upper and lower back.
2. Summarize approaches for managing myofascial pain syndrome, including education, exercises and trigger point injections.

Participants will rotate through two patient stations, where they will be able to talk to patients, examine them and observe two different demonstrations: trigger point injections for 45 minutes and ultrasound for 45 minutes.

Participants will be able to:
1. Take a history of patients with myofascial pain syndrome
2. Find trigger points on physical examination
3. Describe the steps for trigger point injections (consent, position of patient, injection technique)
4. Recognize the sono-anatomy of common neck and low back muscles involved in myofascial pain

Participants will rotate with the nurse educator and occupational therapist to learn about exercises, posture, ergonomics, sleep, relaxation techniques and resources in the community.

Participants will be able to:
1. Review the contributions of the interprofessional team in the treatment of myofascial pain
2. Understand the value of different type of exercise in the treatment of myofascial pain
3. Demonstrate non-invasive, self-management options to release trigger points
