  1. On the Tools menu, click Internet Options, then click the Security tab.

    a. Click Custom Level.
    b. In the Security Settings - Internet Zone dialog box, click Enable for Active Scripting in the Scripting section.
  5. Click the Back button to return to the previous page, and then click the Refresh button to run scripts.

Chrome

  1. In the Chrome address bar, type chrome://settings/content/javascript.
  2. Adjust the toggle so that javascript is allowed.

David Clark, MScOT, OT Reg. (Ont)
Occupational Therapist, Toronto Rehabilitation Institute

David Clark is an occupational therapist at UHN Toronto Rehab, where he works in the MSK Rehab and Multisystem Outpatient Program. He is a hub member and presenter for ECHO Ontario Chronic Pain, and holds a status-only appointment as a lecturer at the University of Toronto’s Department of OS&OT.

Dr. John F. Flannery, MD FRCPC
Medical Director, Musculoskeletal and Multisystem Rehabilitation Program, Toronto Rehabilitation Institute – UHN

John Flannery is a Physiatry graduate from Queen’s University and Medical Director of Toronto Rehab’s Musculoskeletal and Multisystem Rehabilitation Program, University Health Network. His wide range of expertise focuses mainly on complex rehabilitation (amputee, SCI, ABI, burn, polytrauma and peripheral nerve injury) with particular focus in complex musculoskeletal disease and chronic pain during the past 16 years.

Dr. Flannery is Co-Director of the Comprehensive Integrated Pain Program at University Health Network, as well the Operations Committee Lead and Executive Committee Member of Project ECHO – Ontario Pain Program, which is a new initiative in Canada, funded by Ontario Ministry of Health, to help chronic pain patients across Ontario. He is Co-Chair of the University of Toronto Centre for the Study of Pain Education Sub-Committee and sits on numerous other committees, including the Toronto Academic Pain Medicine Institute (TAPMI) and the Committee for Provincial Low Back Pain: Inter-professional Spine Assessment and Education Centres.

He completed the Master Teacher Program through the University of Toronto, Department of Medicine in 2005. In May 2012 he received certificates as a trainer in Crucial Conversations and Difficult Conversations. Academically, Dr. Flannery is a clinician-educator at the rank of Associate Professor in the Division of Physical Medicine and Rehabilitation, Department of Medicine at University of Toronto.

Dr. Andrea Furlan, MD, PhD
Associate Professor, Department of Medicine, University of Toronto; Staff Physician and Senior Scientist, Toronto Rehabilitation Institute

Andrea Furlan is an Associate Professor in the Department of Medicine at the University of Toronto and a Staff Physician and Senior Scientist at the Toronto Rehabilitation Institute. She is also a Scientist at the Institute for Work & Health in Toronto. She obtained her PhD in Clinical Epidemiology from the Institute of Health Policy, Management and Evaluation at the University of Toronto. Dr. Furlan has extensive experience in reviewing the scientific literature for the Cochrane Collaboration and for clinical practice guidelines. She received a CIHR New Investigator Award, and her research focus is on treatments of chronic pain including medications, complementary and alternative therapies, and rehabilitation. She was the team leader for the development of the Canadian Opioid Guideline, and is now involved with Guideline's National Faculty in the dissemination and implementation of the guideline across Canada. She developed the Opioid Manager™, a point-of-care tool for physicians prescribing opioids to help manage chronic pain, which is available in many EMR platforms and as an App for iPad/iPhone. She is the author of the My Opioid Manager, a book and App for patients using opioids for chronic pain. Dr. Furlan is a co-Chair of Echo Ontario(link is external) for Pain and Opioid Stewardship.

Dinesh Kumbhare, MD, PhD, FRCPC, FAAPMR
Associate Professor and Clinician Scientist, Department of Medicine, University of Toronto; Affiliate Scientist, Pain Research Institute, Toronto Rehabilitation Institute

Dinesh Kumbhare is an Associate Professor and Clinician Scientist in the Department of Medicine at the University of Toronto within the Division of Physical Medicine and Rehabilitation. He is an Affiliate Scientist at the Toronto Rehabilitation Institute (TRI). He is cross-appointed to the Institute of Biomedical and Biomaterial Engineering, Faculty of Kinesiology & Physical Education and Institute of Health Policy, Management and Evaluation at the University of Toronto. He is also adjunct faculty in Engineering at McMaster University. He obtained his MSc in Health Research Management from McMaster University and his PhD in Biomedical Engineering at the University of Toronto. He is section editor for the Physiatry Reviews for Evidence in Practice and Resident, Fellow Section with the American Journal of Physical Medicine and Rehabilitation. Dr. Kumbhare was the principal author of the book, Buschbacher’s Manual of Nerve Conduction Studies. According to the Neurodiagnostic Journal, this is “the gold standard in many EMG labs, this manual is a practical working reference for performing a wide variety of common nerve conduction studies. It provides both practicing clinicians and trainees with an impressive database of reference values they can use to interpret nerve conduction results with confidence”. He is leading the Pain Research Institute at TRI, a newly established program that will foster a collaborative environment that brings together multidisciplinary and interprofessional constituency of researchers. TRI is committed to improving the health status of people who suffer from pain. This will be achieved through leadership and excellence in education, delivery of evidence based clinical care, and expanding the horizon of medical knowledge through fundamental science and clinical research endeavors.

Mandy McGLynn, BSc PT, MSc, ACPAC
Physiotherapy Practitioner/Advanced Practice Leader, Toronto Rehabilitation Institute – UHN

Mandy McGlynn, Physiotherapy Practitioner/Advanced Practice Leader / BScPT, MSc, ACPAC

Mandy McGlynn graduated from the Physical Therapy Program at the University of Toronto in 1994. In 2005, she completed her Master of Science degree in the department of rehabilitation sciences at the University of Toronto. Most recently she did the Advanced Practice Clinician in Arthritis Care (ACPAC) course to become a Physical Therapy Practitioner. Mandy has a wide-range of expertise. She has worked in Rehabilitation at Toronto Rehab since 1998. Since 2004, Mandy has been working in the MSK Rehab program as an Advanced Practice Leader (APL). The APL role is a leadership role which is interprofessional and focuses on Best Practice. Currently she is now working 50 percent of the time as an APL and 50 percent as a PT Practitioner.

Nimish Mittal, MD
Assistant Professor, Department of Medicine, University of Toronto ; Medical Lead, Ehlers-Danlos Syndrome Program, University Health Network; Staff Physician - Comprehensive Integrated Pain Program, Toronto Rehabilitation Institute, University Health Network

Nimish Mittal is the Medical Lead of the Ehlers-Danlos Syndrome Program. He is a physiatrist at the University of Toronto with subspecialty fellowship training in chronic pain management. His expertise involves management of complex connective tissue disorders and comprehensive multimodal pain management including USG and fluoroscopy guided interventions. He is a committee member of the Pain Working Group of the International Consortium of the Ehlers-Danlos Syndrome and the newsletter liaison of the regenerative pain medicine special interest group of the American Society of Regional Anesthesia and Pain Medicine. He is also pursuing a Master of Science in Health Research Methodology at McMaster University.

Amy Robidas, RN
Registered Nurse, ECHO Ontario Pain and Opioid Stewardship , Toronto Rehabilitation Institute – UHN

Amy Robidas is a Registered Nurse (RN) and certified rehab nurse currently employed in the outpatient Musculoskeletal (MSK) and Multisystem rehab Program at Toronto Rehabilitation. Her role includes work with patients in all phases of the pain spectrum, from acute to chronic. She is the co-author of the patient guidebook to responsible opioid use ‘My Opioid Manager’ and the accompanying iphone/Android app. Amy has an Honors Bachelor of Health Sciences from the University of Western Ontario and a Bachelor’s of Science in Nursing from the University of Toronto.
