Firefox

  1. In the FireFox address bar, type about:config.
  2. Look for the javascript.enabled setting.
  3. Set it to true by right-clicking and pressing toggle.

Internet Explorer

  1. On the Tools menu, click Internet Options, then click the Security tab.
  2. Click the Internet Zone.
  3. If you do not have to customize your Internet security settings, click Default Level. Then do step 4.
  4. If you have to customize your Internet security settings, follow these steps:
    a. Click Custom Level.
    b. In the Security Settings - Internet Zone dialog box, click Enable for Active Scripting in the Scripting section.
  5. Click the Back button to return to the previous page, and then click the Refresh button to run scripts.

Chrome

  1. In the Chrome address bar, type chrome://settings/content/javascript.
  2. Adjust the toggle so that javascript is allowed.
  3. Close the Settings tab and Refresh the page.

General Rate (after Oct 1, 2019) $250.00
Student, Post-doc, Resident, and Fellow Rate* $100.00

*A valid copy of your student ID, hospital ID or letter from a supervisor must be sent via email to conferences@uhn.ca in order to have the rate honoured.

Please access the online registration form from the "Home" tab of this website.  Registration closes Tuesday October 29, 2019.  

Registration includes continental breakfast, refreshment breaks, lunch, conference material and a certificate of attendance (accessed electronically following the conference).

Please note, if you have indicated that you have a requirement (dietary/accessibility) we will do our best to accommodate this request; however, an additional fee may be applied to the individual if required. Requests will be handled on a case by case basis.

Cheque* – Payable to the University Health Network
*Option available until October 4, 2019. All cheque payments must be received by October 18, 2019.

Cancellation Policy

A full refund, minus an administration fee ($75.00 for general registrations, $25.00 for students/fellows/residents/post-doc), will be issued for cancellations received via email on or before October 18, 2019. No refunds will be issued after October 18, 2019. Refunds will not be granted to attendees who do not attend the conference.
