Stories connect us to each other. Stories are a compelling and meaningful way of sharing wisdom, not just data. Join Kristy Wolfe and Kristina McGuire as they share what digital storytelling is, what is involved in the process and how it can be used within the context of knowledge translation. This interactive session includes digital story examples from a workshop with young adults with Congenital Heart Disease. This will lead to conversations and opportunities to reflect on how your your work could benefit from digital storytelling.

Participants will learn:

  • what a digital story is and why it matters

  • the process of creating a digital story

  • identifying key moments and uncovering lessons

  • the impact of sharing digital stories

The Child Health Hub in Transition to Adult Healthcare is excited to be hosting the 2nd Transition to Adulthood Pop-Up event on December 2-3, 2023, at the beautiful Hyatt Regency in downtown Vancouver. This event aims to bring together youth, families, trainees, researchers, clinicians, and policy/decision makers to celebrate the best of what we are learning, and doing, to help young people to develop fully, even when they are challenged by health conditions. For those unable to join in-person, aspects of the event will be made available in a virtual format.

Participants will hear from local, provincial, and Canadian thought leaders, peers and families. Here’s what you can expect from our event:

- Keynote presentations and facilitated panels with young people and parents.
- Networking is a significant focus of the event. A special community of practice meeting will allow for exchange of ideas and for making connections.
- Showcasing current treatment practices and evidence-based solutions for healthcare transition in Canada through platform and poster presentations, organized by theme – so you get to view content most relevant to you.
