Learn how two co-design teams collaborated to develop interventions to improve discharge communication in pediatric EDs and transitions from pediatric to adult care. Using best-practice methods based on the behaviour change wheel, co-design teams involved youth, parents, and clinicians in a structured approach across a series of virtual meetings. Attendees will gain insights into the value of this collaborative process and how it can improve patient outcomes. Explore the challenges faced and how they were addressed during the co-design process, as well as the outcomes of two rounds of usability testing and how feedback was used to refine the tools.
