Learn about the Extensive Needs Integrated Pathway proof of concept program, a joint initiative between Holland Bloorview Kids Rehabilitation Hospital, Children’s Hospital of Eastern Ontario, and McMaster Children’s Hospital, funded by Ontario’s Ministry of Health and Ministry of Children, Community and Social Services. This program provides specialized and intensive clinical services to children with multiple neurodevelopmental, behavioural, and medical needs, as well as social vulnerabilities. The session will focus on key learnings from the first year of service, including collaborative approaches for multi-organizational partnerships, engagement strategies for key stakeholders, strategies for multi-organizational teams to develop and implement shared models of service access, care delivery, and outcome measurement, and LEAN techniques for rapid program development and refinement. Preliminary outcome data will also be shared, demonstrating the program’s success in reducing barriers to specialized pediatric care.
