D.E. Systems' myConferenceSuite Registration System is responsible for personal information under its control. Collection of personal information by myConferenceSuite will be limited to what is necessary for the purposes of registration for the event.
When we use trusted third parties to act on our behalf by performing such functions as processing credit card payments, contractual or other appropriate means are used to ensure compliance by such third parties with this Policy and all applicable privacy laws.
Personal information will not be used or disclosed for purposes other than those for which it was collected, to process the registration data to a given event.
We will keep your personal information as accurate, complete and up-to-date as necessary for the purposes for which it is to be used.
The Policy is subject to PIPEDA and/or any other applicable privacy laws and myConferenceSuite reserves the right to change it at any time.
Delegate: The person from your co-op who will vote on behalf of your co-op, at both the Ontario Members' Meeting (if applicable) and the National Business Meeting.If your delegate is co-op staff, they would register as "Full CMC" and will have the option to indicate that they will be acting as voting delegate.
There can only be ONE Delegate per co-op, but you may of course send additional co-op members under other registration types.
Alternate: Can vote in place of your delegate should they be unable to attend on Saturday. You must have a Delegate registered before you can register an Alternate.
Additional Participant: (previously “Observer”): Additional, non-voting attendees from your co-op.
CMC: These registration types include managers and all other staff employed by the co-op. You must be Co-op Staff to access the Co-operative Management Conference workshops.
CMC: Co-operative Management Conference
Educational and networking workshops geared towards managers and staff of co-ops.
All CMC workshops begin with the letter "P". The IHM courses P01, P02, and P03 fall under this category.
"Full CMC" - Full registration type which includes: CMC workshops and plenary, Regional Meeting, Friday Dinner, and Saturday AGM/Lunch. Once your co-op registers one co-op staff as "Full CMC", they may register additional staff in the half-price "Additional AMC" registration type.
MEF: Member Education Forum
Educational and networking workshops geared towards members living in co-ops.
MEF workshops will begin with the letters A, B, C or D.
Signing Authority
Is the person at your co-op authorized to sign cheques/issue payments. If
there are multiple people in this role, please choose one name.

CGC: la Conférence sur la gestion coopérative
Les séances sont orientées vers le personnel et la gestion des coopératives.
Tous les numéros des ateliers CGC commencent par la lettre P (ils comprennent les 3 cours IHM ; P01, P02 et
P03).
MEF: Membre Forum de l'éducation
Les séances sont orientées vers les membres vivant dans les coopératives.

La FHCC utilise les renseignements recueillis sur ce formulaire pour vous fournir de l'information et des services liés à notre assemblée générale annuelle. Nous utilisons également ces renseignements pour préparer les rapports statistiques concernant la participation à l'assemblée annuelle pour usage interne.
Sauf mention contraire dans le présent avis, l'accès aux renseignements personnels non publics vous concernant est limité aux employés de la FHCC et aux autres parties qui doivent utiliser ces renseignements pour vous fournir des services. Nous mettrons en place des mesures de protection physiques, électroniques et procédurales conformément aux lois applicables et aux politiques de la FHCC dans le but de protéger vos renseignements personnels contre tout accès ou modification non autorisé.
Votre adresse de courriel sera conservée séparément de toute autre donnée jusqu’à deux ans après cette assemblée annuelle, afin de pouvoir communiquer avec vous concernant votre participation à des sondages et à des consultations organisées par la FHCC dans le but de vous fournir de l’information au sujet d’autres services qui pourraient vous intéresser. Cette information sera sauvegardée sur un serveur sécurisé avec des autorisations restreintes et utilisée seulement aux fins décrites ci-haut.
Tous les autres renseignements personnels recueillis lors de votre inscription à l’assemblée annuelle seront sauvegardés de façon sécurisée avec un accès restreint jusqu’à ce qu’ils puissent être détruits, soit avant le début de l'assemblée annuelle de l'année suivante.

La FHCC fera tout ce qui est raisonnablement possible pour répondre à tous les besoins spéciaux ou à toutes les exigences alimentaires durant notre assemblée annuelle. Ces mesures comprendront, entre autres, la divulgation d'exigences alimentaires spéciales au centre des congrès et/ou au service de traiteur, afin de leur permettre de préparer des menus répondant à toute restriction alimentaire.
Il incombe toutefois à chaque participant à l'assemblée annuelle de déterminer ce qu'il ou elle peut ou ne peut pas manger, et de faire preuve de vigilance pour s'assurer que ce qu'il ou elle consomme satisfait à ses restrictions alimentaires.
En vous inscrivant à l'assemblée annuelle, vous consentez à la collecte, à l'utilisation et à la divulgation de vos renseignements personnels aux fins susmentionnées.
Durant l'assemblée annuelle de la FHCC, des employés de la FHCC ou des photographes contractuels prendront des photographies et filmeront des vidéos. Ces images peuvent être utilisées dans de futurs documents promotionnels imprimés ou électroniques de la FHCC et sur le site Web de la FHCC pour préparer des comptes rendus d'événements de la FHCC et/ou pour promouvoir l'adhésion à la FHCC et les services que nous offrons.
En vous inscrivant à l'assemblée annuelle, vous consentez à ce que votre image puisse se retrouver dans une photographie et être utilisée aux fins susmentionnées.
CHF Canada uses the information collected on this form to provide you with information and services related to the CHF Canada annual general meeting. We also use the information in preparing statistical reports about attendance at the annual meeting, for internal use.
Except as otherwise described in this notice, we restrict access to non-public personal information about you to employees of CHF Canada and other parties who must use the information to provide services to you. We will maintain physical, electronic and procedural safeguards in compliance with applicable law and CHF Canada’s policies to guard your personal information from unauthorized access or alteration.
Your email address will be kept separately from any other data for up to five years after this annual meeting, for use in contacting you with respect to your participation in CHF Canada-sponsored surveys and consultations, and to provide you with information about other services that may be of interest to you. This information will be stored on a secure server with restricted permissions and used only for the purposes described here.
All other personal information collected in your registration for the annual meeting will be stored securely with restricted access until such time as it can be destroyed, which will be before the start of the next year’s annual meeting.
By registering for the annual meeting, you are giving consent to the collection, use and disclosure of your personal information for the purposes identified above.
CHF Canada will make every reasonable effort to ensure any identified special need or special dietary requirement is attended to at our annual meeting. This will include, but not be limited to, disclosing special diet requirements to the convention centre and/or hotel catering departments, to assist them in preparing menus to meet any special needs.
It is up to each meeting participant, however, to determine what they can or cannot eat, and to be vigilant in ensuring that what they consume meets any dietary restrictions.
By registering for the annual meeting, you are giving consent to the collection, use and disclosure of your personal information for the purposes identified above.
During the course of the CHF Canada annual meeting, CHF Canada staff or other contracted photographers will be taking photographs and videos at the meeting. These images may be used in CHF Canada’s future print or electronic promotional materials and on CHF Canada’s website for the purposes of reporting news from CHF Canada events and/or for promoting CHF Canada membership and services.
By registering for the annual meeting, you acknowledge that your image may be taken in a photograph and may be used for the purposes identified above.
